How to Get These Forms The forms on this page are designed to be completed using the free Adobe Acrobat Reader, available for both Windows and Macintosh computers. Search on your computer to see if you already have Acrobat (most computers do have it already). If not, you click on the icon below to download it: To compete the Class Request Card or Registration Financial Form, click on each of the links below. Depending on how you have your browser configured, one of two things will happen: An Acrobat plugin will open in your browser window, displaying the form you requested. You can fill in the form right in the browser (more about that below), and when you are done, print it. Then just click your BACK button to return to this page and get the other form. A download window will open, and you will be prompted about where to save the file that is being downloaded (a .pdf file). Save it to your computer in a place where it will be easy for you to find it. In this case, you will leave your browser and open the saved file, using the Adobe Acrobat Reader, and then fill in and print the form. How to Use These Forms. Whichever way you get the forms on your screen, the process of using the forms is the same. Each space that needs to be filled in is a field. You can start with any field, and then use the TAB key to go to each field after it. Or you can click in any field to go right to that field. If you see a small, solid triangle at the right in any field, it means there is a pulldown menu from which to select. Just click on that triangle, and select from the list that appears. Fill in the forms carefully and completely, and when you are done, click the Print button (in your browser or in Acrobat) to print the form. You should make an extra copy for yourself, because Acrobat Reader does not allow you to save a copy of what you have filled in. Sign the forms, and bring or send them, along with your payment, to your local Extended Education office. Important Note. To complete the financial worksheet, you need to know your student account balance, if any. Just log into Campus Pipeline and click on the My Student Records link on the right hand side of the first page.
