Blue Cross of California (Power Select HMO) - Fall 2007 through Summer 2008

Blue Cross of California is the HMO option for eligible Fuller Pasadena Campus students who reside in Southern California, which covers preventative care along with injuries and illnesses. Students who are required to have health insurance are eligible to purchase this insurance.

Be Informed Before You Buy (PDF)

Blue Cross Health Insurance Form (PDF)
Required every quarter for all eligible students who wish to purchase Blue Cross insurance.
Blue Cross Enrollment Form (PDF)
This form should be submitted in the Fall quarter or the first quarter that a student purchases Blue Cross during the academic year, whichever comes first.  After printing the document, please complete the form and verify that your information is complete.  Please be sure to sign the form.  You may fax your completed form to the Student Health Insurance office at 626-204-2072.


 Health Insurance Cancellation Form (PDF)

  • This form is for students who meet the eligibility criteria for cancelling their medical and/or dental insurance by the quarterly deadline.

  • To waive insurance if you have comparable coverage, go online to www.renstudent.com/fuller to complete the process.

  • Submit this form and proper documentation to the Student Health Insurance Office by the applicable quarterly deadline.
